Transplant Rider
Effective 10.01.07, ICSVEBA is pleased to announce the implementation of AIG's Transplant Rider. This benefit is for those members needing an organ transplant.
The benefits are as follows:

$2 million (in addition to and not counted towards your $5 million lifetime max under your medical plan) |

Evaluation through 365 days post transplant |
| In Network |
100% of ALL covered expenses, including organ (AIG Network; not Blue Cross. A list can be requested) |
| Out of Network |
80% of ALL covered expenses up to maximum |
| Transportation |
$200/day - $10,000 max for patient and companion |
| Experimental |
NCI Clinical Trials: Phase III & IV for adults and All for pediatric. |
| Pre Existing |
If member has been evaluated, recommended or in waiting period 24 months prior to 10.01.07, then excluded from this benefit for first 12 months. |
