Flu Season
During flu season, your ICSVEBA Plan covers flu shots for you and your covered dependents for a $0 copay if you visit a participating Express Scripts pharmacy. For a list of participating pharmacies, please visit the Express Scripts website by clicking the link below.
You can also access the list by using the mobile app. For both the website and the mobile app you will need to register to access the list.
VITALITY HEALTH (EFFECTIVE 10.1.23)
Vitality will replace Go365 as your wellness and rewards program effective 10.1.23. All ICSVEBA members are eligible to participate and register on their Vitality account. You will receive your Vitality ID card in the mail upon becoming an ICSVEBA member, that has your very own ID number on it that you will use to register. Once registered, get out of the Blue status by completing at least 1 of the 3 following ways: 1) Complete 1 section of the Health Assessment on PowerofVitality.com or the mobile app, 2) Get a bio-metric screening, or 3) Log a verified workout. Please see your Human Resources Department to get more information on how to enroll or for any questions you may have. Remember you must have your Vitality ID number first in order to register.
CCS Living Connected Diabetes Program
ICSVEBA members have access to a new Diabetes Program through CCS Medical. They will provide you high-quality products and support you with personalized service. Diabetes supplies will be provided at 100% coverage with no out-of-pocket costs. Supplies such as state-of-the-art cellular and Bluetooth glucose meters, test strips, lancing devices, and lancets are available. They offer free home delivery of supplies as you need them, with no out-of-pocket expenses, alerts and coaching from a CCS nurse when your readings are out of range, live ongoing personalized health coaching from Certified Diabetes Educators and 24/7/365 toll-free access to live nursing support! Please note that participation in the CCS Living Connected Program is voluntary and you must actively enroll to participate.
